think u got it right. But for the optimal sound you play the A's and the C's as 7ens like this
A = 5-7-5-6-5-5 or - 0-2-0-2-0-0
C = 3-3-5-3-5-3 or - 0-3-2-3-1-0

Good tab, but I think it might be better to go back to A instead of using the D chord
